New Patient Steps
1) If you are interested in scheduling your initial evaluation with Dr. Gupta, the first step is to confirm your insurance coverage and view the Accepted Insurances tab to see what plans we take, as well as our self-pay options.
2) The next step is to complete our New Patient Intake using the link Click here to set up an appointment! located at the top of the website.
3) After submitting this form, the doctor will review your information to ensure that he is the best match for your needs. You will be called within 48 hours to get any additional information needed to schedule the appointment.
Scheduling
We do our best to schedule new patients as soon as possible, with most appointments being made within 1-2 weeks.
The Office Hours are as follows:
Monday: 9:00AM – 7:00PM
Tuesday: 9:00AM – 7:00PM
Wednesday: 9:00AM – 7:00PM
Thursday: 9:00AM – 7:00PM
Friday: 9:00AM – 5:00PM
Saturday: 9:00AM – 1:00PM
Sunday: Closed
